Hospice RN, York/Adams County

SpiriTrust Lutheran is now hiring an RN for Home Health and Hospice at our facility in York/Adams County, PA.

Required:

  • Graduate of a school of nursing with current license(s) to practice as a professional nurse in the state(s) served by SpiriTrust Lutheran Home Care & Hospice
  • Current CPR certification within three (3) months of employment
  • A passion for serving older adults

Experience: One (1) year of recent clinical experience in licensed or certified discipline normally required.

General Responsibilities:

  • Assesses and evaluates the client’s need for Skilled nursing services.
  • Travels to client’s location in various weather conditions.
  • Gains access to the client by available means which may include but is not limited to steps; uneven surfaces; wet, snow covered or icy surfaces; narrow entrances; etc.
  • Assesses visual, olfactory, tactile and auditory signs and skilled nursing needs of the client.
  • Identifies and prioritizes nursing diagnosis and determines realistic goals in cooperation with client/ family and/or attending physician.
  • Provide skilled nursing services and manage assigned client cases.
  • Coordinate work within the department and collaborate with other departments
  • Ensure compliance with operating policies, procedures, and regulatory requirements.
  • Communicate effectively with appropriate personnel and respond to inquiries
  • Maintain accurate records and submit assigned reports in a timely manner.
  • Visit clients in their homes and facilities, particularly during end-of-life care, to monitor signs and symptoms.
  • Attend interdisciplinary team (IDT) meetings and provide comprehensive reports on all assigned clients.
  • Conduct weekly visits to clients and ensure all documentation is submitted within 24 hours of each visit or admission.
  • Perform client admissions and participate in on-call rotations, including weekend work as required.
